How to Send Your Prescription

Important Prescription Information

Prescriptions must include:

  • Patient name and date of birth

  • Medication details including strength, dosing, and form

  • Provider signature and contact information

  • Allergies

Use Our Prescription Forms

Coming Soon: Download a prescription form from our website and take it to your provider to complete.

Once completed, the prescription can be:

  • Faxed to our pharmacy

  • Dropped off in person

Have Your Provider Send It

Providers can fax prescriptions directly to:

If you do not currently have a provider or are looking for a more comprehensive approach, you can become a patient of Megan L. Miller Direct Primary Care.

This allows for:

  • Full evaluation

  • Prescribing

  • Ongoing management

Providers can electronically send prescriptions directly to RxAvenue for processing.
